For western meal the proper etiquette would be to be seated from the right side of the seat entering with the left side of the body. Never toy with the napkin which should be opened and spread on the lap, with your hands beneath the top of the table and out of sight from the others at the table. The fork and knife to be used will be in the order from the outside to the inside.
When halfway during the meal you need to pause to go to washroom you place the fork and knife apart with the tip pointing at the 12:00 clock using the plate as a clock face. Once you complete your meal you should place them at 4:20 position assuming the plate is a clock face.
